ADCES Awards

Each year at the ADCES Annual Conference, we recognize excellence in diabetes care and education and honor members' service to the association and the larger diabetes community.

ADCES offers a variety of awards and scholarships to celebrate those individuals who have gone above and beyond.

Allene Van Son Distinguished Service Award

This award recognizes outstanding volunteer and leadership contributions and service to the Association of Diabetes Care & Education Specialists. This award is named in honor of Allene Van Son Martinez, ADCES’s first president and leader in diabetes care. Van Son Martinez was a lifetime advocate for those affected by diabetes and a champion for the Association. Van Son Martinez was the first recipient of the Allene Van Son Distinguished Service Award, named in her honor to recognize one diabetes care and education specialist each year for outstanding contributions and service to the Association.

Award

The award recipient will receive a complementary registration to the ADCES Annual Conference, where they will be recognized and a $1,000 travel stipend. The award recipient will also be given a Lifetime Membership. Annual Meeting may be in-person, virtual or a combination.
Eligibility

Applicant/Nominee must be, for at least five years, an active member of ADCES as defined by the ADCES Bylaws, including at the time of submission and at the time of receipt of the award. Current members of the Board of Directors and past recipients are not eligible.

Advancing the Vision Award

Recognizes a member whose program or project advances the ADCES Project Vision strategic pillars to achieve the outcomes inspired by ADCES’s mission.

Award

Recipient will be recognized at the ADCES Annual Meeting with a plaque, will receive a complimentary registration to the Annual Meeting and one-year Active Membership. Annual Meeting may be in-person, virtual or a combination.
Eligibility

Applicant must be an active member of ADCES, as defined by the Association’s Bylaws, for at least three continuous years at the time of submission and receipt of the award. Past recipients are not eligible to apply for this award.
Criteria
Provide a description of your program or project. This description should include goals and objectives, methods, program's needs, and expected outcomes. Describe how your project or program reinforces the ADCES Project Vision strategic pillars (see above). Please describe how your project or program aims will achieve the outcomes inspired by ADCES's mission.
Applications must meet the following requirements to be considered:
- The educational project or program for use in practice must reinforce the ADCES Project Vision strategic pillars
- The education project or program must be developed for use in practice DSMES

Innovative Use of Technology Award

Supports the innovative use and integration of diabetes management and/or education software and/or technology practice. The project submitted should have significant impact on the assessment and provision of personalized prediabetes, diabetes and related cardiometabolic conditions and self-management interventions that enhance or improve practice for individuals or broad populations.

The proposed project must be completed within twelve months of funding to be eligible to present at the ADCES Annual Meeting and Exhibition the following year. The recipient will be required to submit an abstract for the 2023 Annual Conference.

Award

Recipient will receive complimentary registration to the next ADCES Annual Meeting, where they will be recognized as the recipient of the award. Annual Meeting may be in-person, virtual or a combination.
Eligibility

Applicant must be, for at least one year, an active member of ADCES, as defined by the Association’s Bylaws, at the time of submission and receipt of the award. Should there be co-developers, they should also be members of ADCES.

The proposed project must be completed within twelve months of funding to be eligible to present at the ADCES Annual Meeting and Exhibition the following year. The recipient will be required to submit an abstract for the 2024 Annual Conference.
Criteria
Recipient must present a podium session or poster at the Annual Meeting. This will assist in providing more information to members about the award and the importance to the practice of self-management.

Submissions for this award will be reviewed according to the following criteria:
- Significance of the innovative use and integration of diabetes software and/or technology in enhancing practice
- Potential to affect large populations
- Clarity, feasibility, sustainability
- Potential for broad based application, other disease assessment and intervention programs to use project/tool/strategy in their practices
- Alignment with ADCES mission and strategic goals

Applications that do not meet the following requirements will not be considered. The applicant must provide a narrative description of the project, target audience, purpose and outcome measures. Application materials must include a completed Cover Page which can be uploaded at the end of the application, or the application will be disqualified.

The educational tool/program for use in practice must be original in design.

The project should be developed by the individual submitting the application; however, it can be a team project. Please describe your role on the team and provide information for other team members.

The tool/program may not be developed by a commercial firm or national organization.

The tool/program must be developed for use in practice of DSME and support.

Diabetes Care and Education Specialist of the Year Award

Honors a diabetes care and education specialist who has made a special contribution to the field through dedication, innovation, and sensitivity in patient care. The Diabetes Care and Education Specialist will act as an "Ambassador" for ADCES and should be someone who is considered a role model by their peers and colleagues.

Award

Recipient will be recognized at the ADCES Annual Meeting with a plaque, will receive complimentary registration to the Annual Meeting and a $5,000 award. Annual Meeting may be in-person, virtual or a combination.

The recipient may select either the current five virtual/face-to-face educational sessions in which he/she is asked to travel, if not a virtual session, to Coordinating Body or Local Networking Group meetings and present an educational session or select the option to present 1-2 webinars and one session at the Annual Meeting, Virtual or In Person, in the year immediately following his/her selection.
Eligibility

Applicant/nominee must be, for at least five years of continuous membership at the time of application, an active member of ADCES as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award. Current and past members of the Board of Directors and past recipients are not eligible.

At the time of the application submission, the candidate must be a Certified Diabetes Care and Education Specialist or BC-ADM for at least 5 years.
Criteria

Candidates for the Diabetes Care and Education Specialist of the Year may be nominated by a peer/colleague or by self-nomination.
Application Process
The nominator/applicant must submit

Three letters of support, of not more than 1,000 words, from a professional colleague other than the nominator. To include:
- How you know the nominee
- Why you are supporting the nominee for the Diabetes Care and Education Specialist of the Year Award
- How the nominee has made a special contribution to the field of diabetes education through dedication, innovation, and sensitivity in patient care
CV or Resume

Applications and supporting materials must be submitted by March 22, 2022. All applications must be complete to be considered. Please provide as much detail as possible in your answers. Applications demonstrating thoughtful, detailed responses tend to yield more favorable results. If selected, what would be your topic or platform.

Research Award

Recognizes noteworthy translational scientific work that addresses the organization's vision and advances knowledge in the delivery of diabetes care and education. The research should include an ADCES-related research priority (diabetes technology, telehealth, implementation science, health equity, peer support, ADCES7 self-care behaviors, precision medicine, data science, innovative DSMES services, or community resources and policies). The recipient will receive conference registration and a $500 travel award.

Eligibility: Research abstracts submitted for the annual conference can be considered for the award. The abstract's first author or the primary conference presenter is eligible for the award. The research should be submitted to and/or published in a peer-reviewed journal, with the applicant being the PI or Co-I of the study or demonstrating a significant contribution to the research design or analysis.

Rising Star Award

Recognizes a diabetes care and education specialist who has less than five years of diabetes care and education experience and has demonstrated leadership and commitment in their practice.

Award

Recipient will be recognized at the ADCES Annual Meeting with a plaque, will receive a complimentary registration to the Annual Meeting and one-year Active Membership. Annual Meeting may be in-person, virtual or a combination.
Eligibility

Nominee/Applicant must be an active member of ADCES, as defined by the Association’s Bylaws, including at the time of submission and at the time of receipt of the award with less than five years of diabetes care and education experience and has demonstrated leadership and commitment in his/her practice.
Criteria
Applicants for the Rising Star Award may be nominated by a peer/colleague or by self-nomination. Applicants must be an ADCES member who is in active practice of diabetes self-management education and support (DSMES) with no conflict of interest. Applicant should be an early careerist, with less than five years in the field of diabetes care and education.
An appropriate applicant meets the needs of patients by using effective practices which may include:
- Innovation and use of new approaches to the delivery of DSMES (description of the role the applicant played and the resulting positive outcomes that have occurred using innovative processes/approaches)
- Working creatively in applying established and new scientific knowledge in DSMES
- Demonstrates leadership skills and/or committed service to the practice (leadership position in LNG, CB, COI or ADCES national committee). The applicant's portfolio of leadership roles have resulted in increasing levels of responsibility.
- Fosters community connections (JDRF, ADA, AHA, YMCA, etc.). Evidence that the contributions the applicant has made to community connections are sustainable and have a positive impact on the community and/or the profession.

The nominator/applicant must submit:
- Two letters of support, of not more than 1,000 words, from a professional colleague other than the nominator

Donna Tomky Award for Excellence in Clinical Practice

This award will be given in memory of Donna Tomky, an ADCES Past President and Nurse Practitioner (NP), who advocated for the integration of self-management behaviors with clinical management, to a NP who continues to advance this integration through practice, health plan or health system innovation.

- Recipient is recognized at Annual Meeting each year.
- Recipient will receive complimentary registration to the ADCES Annual Meeting and up to $1,000 in travel expenses (subject to change).
- Annual Meeting may be in-person, virtual or a combination.
Eligibility
- Nominee is a Nurse Practitioner.
- Nominee is a member of ADCES.
- May have a CDCES or BC-ADM credential but not required.
Criteria
- Must focus on the integration of self-management with clinical management
- Incorporates ADCES7 Self-care behaviors
- Optimizes clinical management/treatment
- Uses technology to impact care
- Patient-centered
- Addresses health care delivery
- Can be a tool, a technique, a practice intervention
The nominator/applicant must address in writing how the nominee meets the criteria stated above in 500 words or less (essay).

The Power of Our Impact Award

Recognizes a diabetes care and education specialist for their on-going dedication and service to persons with diabetes and the impact of diabetes self-management and clinical care that diabetes care and education specialists provide. The recipient of this award would be honored for their day-to-day commitment to clinical care to ensure the best outcomes for persons with diabetes. This award provides an opportunity for ADCES to recognize members in the field for their dedication to persons with diabetes who work every day to improve the quality of their patients' lives.

Recipient will be recognized at the ADCES Annual Conference, receive complimentary registration to the Annual Conference, a one-year active membership, and $1,000 travel stipend.
Eligibility

Applicant/nominee for this award will need to have completed a minimum of ten (10) years' experience as a diabetes care and education specialist at the time of submission of the application. Applicant/nominee will need to provide examples of collaboration between the applicant/nominee (working in an inpatient or community setting for example) and the person with diabetes using a unique strategy or intervention which empowered the person with diabetes to make concrete behavior change, resulting in improved outcomes.

Applicant/nominee must be, a member for at least three (3) years of continuous membership at the time of application, an active member of ADCES as defined by the Association's Bylaws, including at the time of submission and at the time of receipt of the award. Current and past members of the Board of Directors and past recipients are not eligible.

Applicant/nominee must submit a CV or resume along with two (2) letters of recommendation, of not more than 1,000 words, from a professional colleague other than the nominator, to include:

ADCES/Children with Diabetes - Friends for Life Fellowship Program

ADCES seeks to provide an opportunity to a diabetes care and education specialist, to learn about Type 1 diabetes from the perspective of the patient, family, and caregiver. Funding provided by the ADCES Foundation.

Award

This award recipient will receive complementary registration, housing and travel-related expense reimbursement to attend the Children with Diabetes Friends for Life Conference in Disney's Coronado Springs Resort, Lake Buena Vista, Florida on July 5-10, 2022.

This fellowship exposes a member to children and families who are affected by type 1 diabetes, and mirrors a fellowship offered by the American Association of Clinical Endocrinologists which seeks to develop professional understanding and strengthen the relationship between endocrinologists and diabetes care and education specialists.
Eligibility
- ADCES Membership - Active membership at the time of application submission and receipt of the award
- Should work with Children with Type 1 diabetes
- CV or Resume attached
- Letters of Support - 1 letter required
